Independent Voices — Allied Physician & Pharmacist
Guardians of Medicine
Dr. Joseph Miladore, MD · Dr. Anthony Fossaceca, PharmD · Northeast Ohio
A doctor and a pharmacist — both sides of the prescription — formerly Guardia di Medicina
Why They're Here
Guardians of Medicine puts a family physician and a clinical pharmacist at the same table. One writes the prescription; the other fills it. Between them they see the prior authorizations, the formularies, the reimbursement cuts and the corporate consolidation from both ends of the counter — and they say out loud what most clinicians keep quiet.
"Our mission is to be a guardian for the public, shedding light on the systems and policies that impact your health every day."

Where Their Work Meets the Map
June 1, 2026 · AI & Prior Authorization
When AI Starts Deciding Who Gets Care
AI can catch cancer earlier and predict heart failure — but every system becomes what it optimizes for. Built for cost reduction, it turns "a frightened mother, a dying grandfather" into spreadsheet variables. Their fork in the road: corporate AI controlling care from above, or physician-led intelligence protecting patients from below.

Inside the Business of Medicine
With Greg Davis, founder of Patriot Home Health. How conglomerates that own the insurer, the PBM and the physician groups squeeze independent providers — and why audits chase clerical errors by honest agencies instead of the real bad actors.

This Is Why Healthcare Fails Us
Patients denied coverage get angry at the doctor in the room — but the doctor didn't design the formulary or the prior authorization. Their argument: doctors and patients aren't enemies. They're both trapped inside the same corporatized system.

Luigi Mangione & Our Healthcare Crisis: What's Really Going On?
The hosts refuse to condone violence. They ask why so many Americans are this angry: about $2,000 a month in premiums with thousands more in deductibles, no way to know the price before care, cuts to preventive-care payments, and a family member's oncologist-recommended treatment fought over by insurers.

Microplastics, Forever Chemicals & the Cover-Up
Dec 29, 2025 · From the Guardia di Medicina era
Plastics and forever chemicals in blood, lungs and placentas; endocrine disruption; "regrettable substitutions" like BPA swapped for BPS under a "BPA-Free" label; and the same few institutional investors holding stakes in both the chemical makers and the healthcare giants. The same record AbilityForge documents, from a doctor and a pharmacist.

Can Science and Christianity Agree?
With Pastor Chris from Old North Church: science and Scripture as partners, not enemies — and where the ethical lines fall on gene editing and brain implants.
"There was a time where I was so committed to the US healthcare system that I was married to it… I gave my blood, sweat, and tears… Let's bring Jesus closer to medicine."
